Personal Data Sheet (PDS) CS Form 212 (Revised 2017)




GUIDE TO FILLING OUT THE PERSONAL DATA SHEET (PDS)


Warning:

Any misrepresentation made in the Personal Data Sheet and the Work Experience Sheet shall cause the filing of administrative/criminal case/s against the person concerned. 

Please fill out each of the fields in the PDS when applicable. 

Note:
The PDS may be accomplished using the MS Word format or MS Excel format.

In the MS Excel format, all the tick boxes will automatically be marked once clicked.

The PDS must bear the signature of the employee and date of accomplishment at the bottom of every page.

Entries in the PDS may be filled out through handwriting or via typewriter/computer.  If handwritten, entries should be in block capital (e.g. PRINT) format using a pen.  

All information should be provided accurately.

Do not leave blank entries.  Put N/A if not applicable.
For purposes of application to a vacant position, the additional sheet for work experience should be accomplished.

I. Personal Information

Employee’s name is to be filled out in the following format:  surname, first name, name extension (if any), middle name.  A space is allotted for each character or letter in the name.
Dates are in numeric format: mm/dd/yyyy
Specifics should be given to “Others” response in the civil status field.
Agency employee number refers to employee ID number in the current agency.
For holders of foreign/dual citizenship, please select from the dropdown list the foreign country where you were born/naturalized or type/write the same in the space provided therein.

II. Family Background

Names of spouse and parents are to be filled out the following format: surname, first name, name extension (if any), middle name.
Mother’s name is her maiden name, or name when she was single or before marriage.
List full names (first name and surname) of ALL your children.
Date of birth is in numeric format: mm/dd/yyyy

III. Educational Background

Indicate FULL name of schools.  DO NOT ABBREVIATE.
For Elementary Level, indicate PRIMARY EDUCATION if graduated 
For Secondary Level, indicate HIGH SCHOOL if graduated under the old curriculum; or JUNIOR HIGH SCHOOL or SENIOR HIGH SCHOOL if graduated under the K-12 curriculum.
Indicate in FULL all courses taken in college (e.g. ASSOCIATE IN ARTS, AB ECONOMICS, BS PSYCHOLOGY, MA IN HISTORY).
Indicate all masters or doctorate degrees taken.
If graduated for every level, indicate year of graduation.
If not graduated in any level, indicate the highest grade, level or units earned.
Period of attendance are stated in school years (e.g. 1992-1996)
Indicate any scholarship and/or academic honors received in each level.

IV. Civil Service Eligibility

Indicate all civil service eligibilities earned with corresponding rating, date and place of examination/conferment.

Example:

Career Service Sub-Professional EO132/790 – Veteran Preference Rating
Career Service Professional PD 907 – Honor Graduate
Career Service Executive RA 7883 – Barangay Health Worker
Stenographer Barangay Official 
PD 997 – Scientific and Technological Specialist

If earned eligibility entails a license (RA 1080), indicate the license number and its date of validity.

V. Work Experience

Indicate all positions held both in the public and private employment starting from current work.
Inclusive dates are indicated in numeric format: mm/dd/yyyy.
Indicate FULL position titles and COMPLETE NAME of department/agency/office/company.  DO NOT ABBREVIATE.
Indicate monthly salary in figures (e.g. P21,877).
Salary grade and salary step, if applicable, should be stated in the format ”00-0” (e.g. 24-2, 24 for salary grade, 2 for salary step)
Indicate status of employment (e.g. permanent, temporary, casual, contractual)
Indicate “yes” under government service if position held is in the public or government employment or “no” if held in the private employment.
Additional sheet for work experience should be accomplished and submitted together with the PDS in case of application to a vacant position. This should be accomplished only for work experience relevant to the position being applied to.

VI. Voluntary Work or Involvement in Civic/Non-Government/People/ Voluntary Organizations

Indicate the FULL name and address of the organization where involved as voluntary worker.
Inclusive dates, start (from) and end (to) should be in numeric format: mm/dd/yyyy.
Indicate the number of hours of voluntary work rendered.
Indicate the position/nature of voluntary work rendered.

VII. Learning and Development Interventions

Indicate FULL titles of learning and development (L&D) interventions attended during employment.  Indicate list from the most recent L&D.
Inclusive dates of attendance, start (from) and end (to) should be in numeric format: mm/dd/yyyy.
Indicate the number of hours attended for program.
Indicate the type of L&D intervention (e.g. managerial, supervisory, technical).
Indicate the FULL name of institution/agency that conducted or sponsored the program.  DO NOT ABBREVIATE.  (e.g. CSC should be Civil Service Commission).
  
VIII. Other Information

Indicate special skills /hobbies.
Indicate in FULL non-academic distinctions/recognition (awards received)
Indicate membership in any professional association/organization by writing in FULL said association/organization.

Indicate response to questions 34 to 40 on the right side of the sheet.
Provide details or specifications for any yes response.

Indicate the FULL name of references with the format FIRST NAME, MI, SURNAME, their addresses and respective telephone numbers.

As agreement to and for completion of the PDS, the employee’s signature and right thumb mark should be affixed in the boxes provided.  Indicate also the government ID number and date of issuance in the boxes provided.  Lastly, attach a RECENT PASSPORT SIZE (4.5 cm. x 3.5 cm.) picture with COMPLETE AND HANDWRITTEN NAME TAG and SIGNATURE OVER PRINTED NAME. Picture must be taken within the last six (6) months. Computer generated or photocopied picture is not acceptable.

National Achievement Test (NAT) Result

Although it is a 'Herculean Task' for us to reach a very satisfactory MPS in NAT-Science IV due to a very large population of students, this year we've made it to the 3rd out of 9 Public Secondary Schools in Parañaque Division.

1st     ParScie               91.01
2nd   SVNHS              60.77
3rd    PNHS-Main       53.51

ParScie made it to the top with a very high MPS reflecting the mastery level of their students. Although we've made it to the 3rd, still our MPS is below mastery level. The good thing is, based on the 3-year Comparative Result in NAT-Science IV in our Division, this year we've made a great increment from our previous NAT.

What is NAT?
Taking National Test

The Department of Education (RP) through its National Education Testing and Research Center (NETRC)
conducts an Annual National Test for selected grade levels. This is called the National Achievement Test or NAT.

Since S.Y. 2011-2012, NAT was administered to Grades 3 & 6 of Elementary Level and 4th year for Secondary Level.

What is Open High School Program (OHSP)?

With the tagline, "Reaching the Unschooled Wherever and Whenever," Open High-School is one of the newest program of the Education Department to reach out most of our out-of-school youths.

This School Year I'm one of the fortunate teachers who were assigned to Open High School Program (OHSP). This is another teaching load aside from our regular classes. Based on the orientation for teachers, this year marks the 4th year of implementation of the said program in our school.

So what is OHSP?

Open High School Program or OHSP is an alternative mode of formal secondary education program run by the Bureau of Secondary Education (BSE) of the Department of Education (DepED) of the Republic of the Philippines. This program provides an opportunity for elementary school graduates, high school drop-outs and successful examinees of the Philippine Education Placement Test (PEPT) to complete secondary education in a purely distance learning mode. The program provides printed self-learning modules for students to use for their lessons and classroom activities.


Qualifications for students who wishes to avail of OHSP:
•     Filipino citizen
•     Independent learner
•     Have completed the pre-requisite grade or year level
•     Must pass the Independence Learning Readiness Test (ILRT) and the Informal Reading Inventory (IRI)

OHSP is available anytime. It is not dependent on the calendar of the regular school year. Student may start at their own time and at their own pacing, provided they complete their entire high education within a maximum period of six (6) years.

OHSP is intended for students who would otherwise be unable to pursue formal secondary education due the rigid requirement of regular class schedules. It is an alternative to attending regular classes but is not designed to be beneficial to all students. Most students may still learn best by attending the regular formal secondary education.

Summer Classes???

In my seven years of teaching in public secondary school, I still considered myself a newbie in may aspects of my profession. When I was hired in public school, I was a substitute to a teacher (who was then promoted to principal after passing the exam for school heads) with teaching loads assigned to middle sections. The following school year, I was given a teaching load with the lowest sections. To sum it up, my limited experience, exposed me to different types of learners. From the lowest sections to the highest sections, from academically inclined students to passive students, from good-mannered students to students with attitude problems...... I want to believe that I have already met all types of learners not until I've experienced teaching Summer Class.

This year I was given a chance to teach Summer Class. At first, I was actually hesitant to apply because of many stories I'm hearing about their experiences. What had driven me to do so was my urge to have a first hand experience teaching Summer Class and this is my chance. This is because in our Subject, we had an agreement (among teachers) to have a rotation for application to teach Summer Class. There are 13 teachers in our subject area, so if I let go of this chance, it would be another 13 years after I will be given another chance.

 After submitting an Application Letter to the Principal, the applicants were called for a meeting. The schedule were given, it is 6 times a week, Mondays to Saturdays, 3 hours per subject, a total of 36 school days.

This is my first time! And I'll try to answer the following questions based on my experience.

Last Saturday, we held our Commencement Exercises for graduating 4th year students this Summer Class. Almost a hundred students successfully graduated from our school & annexes.

Values Integration in every lesson, specially for Learning Areas other than Values Education itself is a vital part of teaching --- whether the DepEd or Educational Theories prescribed it or not, it is actually already existing in the classroom scenario. As a matter of fact - even us, during the time when we were also students, we were bombarded with such 'sermons' by our teachers then.

I'm just actually justifying the picture that I'll be showing below... hahaha. After graduation, some of my students posted pictures discretely on their group - these are stolen pictures of their teachers while inside the classroom. They have done this not for anything else but just for fun.

One throwback picture posted by my students which made me smile was about me solving Life as a mathematical equation. I have seen this equation circulating in the social networking site and I cannot remember what particular instance did I inserted this in our discussion.

Life is 1/2 Happy plus 1/2 Sad

4-day school to solve traffic???

My Dept. Head handed me a survey-questionnaire about the proposed 4-day class a week to solve traffic problems in Metro Manila.

And because of that, I've asked the opinion of my co-teachers present during that time in the faculty. We have different views - a lot of pros and cons were raised and it ignited a healthy discussion.

In my own opinion, as what I've written in the survey-questionnaire, and I quote;



We should not sacrifice the quality time of our Education just to solve traffic problem. I strongly believe that the Education sector should not be the one to suffer just to solve this problem. I believe that 5-day contact time is properly equated to the attention span of our students nowadays & increasing the duration per session to compensate for a 4-day class schedule will 'worsen' the quality of our education.




Although, some of our Education officials are still thinking about alternative delivery mode of instruction to compensate for the contact time lost, let's just hope that this will not put at risk the education of our students.

What is Project DAHON

Project DAHON: An Advocacy

 
What is Project DAHON?

Launched Date: 18 December 2013.

Project DAHON stands for Developing Awareness as Human Obligation to Nature. It is an advocacy aiming for a greater impact on the lifestyle of every energy consumer and carbon dioxide emitters.

Specifically this advocacy aims to;

1. develop functional awareness about energy efficiency and conservation
2. mold responsible energy consumer
3. promote effective and realistic solutions in mitigating the effects of climate change
4. adapt best practices in energy conservation



Our Commitment

The big word today is CLIMATE CHANGE... and we have to do something about it, before it's too late...

This project was conceptualized in response to the training conducted by the Center for Sustainable Human Development - Development Academy of the Philippines (DAP) in collaboration with the Department of Energy (DOE) and Department of Education (DepEd) held on Hotel Kimberly, Tagaytay City last November 28-30, 2013, entitled  Facilitators' Training on Energy Efficiency and Conservation: Usapang Climate Change. The teachers who attended the above-mentioned seminar will serve as the Advisers of the Committee.

Project DAHON is an environmental awareness campaign in which the main role players are high school students through the guidance of their Science Teachers (Advisers). The ultimate goal of the Project is to develop the value of environmental awareness to students at their young age. The academe is the best venue for this goal because of its multiplier effect. Every little act done for the environment counts and we must practice it for it to become a habit and eventually a way of life.

Project DAHON Committee works hand-in-hand with the existing Science Organizations in the School such as the Youth for Environment and School Organization (YES-O) and National Ecosavers Program (NEP). The success of the launching of Project DAHON becomes possible primarily because of the support of our School Head, Science Supervisor, Science Department Head, Science Teachers (PNHS-Main) and of course the warm acceptance of the students.

Project DAHON: Looking Back

Greenphysics then...

As early as 2009, Science Teachers (Science Department of PNHS-Main) have already been promoting our commitment to our environment. 

Prior to Project DAHON, in my handled sections of senior students I've introduced 'greenphysics.' Greenphysics is an advocacy for our environment which started in the 2nd week of December 2009 in response to a directive to integrate Environmental Lessons to all Science subjects (including Physics). This was a collaborative effort of the students with the guidance of their Science Teacher with the ultimate goal of DOING something (not just talking) FOR OUR ENVIRONMENT. 

Our advocacy then was called 'Greenphysics' primarily because we have integrated Environmental Lessons to Physics topics.This advocacy was promoted not only in my science classes but also online through greenphysics.multiply.com. (This address has already expired after multiply.com ceased to operate.)  The scope of 'Greenphysics' is limited only to my handled sections during that time. Students working in groups (composed of 6 to 7 members) were instructed to find a suitable place to plant one tree per group. They have planted during the 1st Semester of the School Year and as their output, they have to write a blog monthly to give an update of the growth of their tree as well as their experiences in doing such. The name of their blog is also their 'group name' and each blog entry must be accompanied by a picture of their tree.

Last November 28-30, 2013, the Center for Sustainable Human Development - Development Academy of the Philippines (DAP) in collaboration with the Department of Energy (DOE) and Department of Education (DepEd) held a training about Energy Efficiency and Conservation (Facilitators' Training on Energy Efficiency and Conservation: Usapang Climate Change) at Hotel Kimberly, Tagaytay City. The training was attended by 4 Science Teachers in our Division, 2 teachers were from our school.

In response to the said training, each participating teachers from different schools were tasked to create a Work Plan on how to roll-out the Module presented during the training. As part of the plan of rolling out the material in our school, we have created a working group that will make the task identified in the work plan possible. This marked the birth of Project DAHON. The main objective of the project is a 'functional awareness campaign' for our environment.
